Healthy Environments Active Living (HEAL) is a program within the state Department of Health and Human Services (DHHS). Formed in July 2013 (as EPICC) through CDC funding, it merged the Heart Disease and Stroke Prevention Program, the Diabetes Prevention and Control Program, the Physical Activity, Nutrition and Obesity Program, and a school health program. HEAL was restructured to address social determinants of health, advance health equity, and increase policy, systems and environment changes. Vision: All residents have equitable opportunities to lead healthy, informed, safe, and productive lives. Mission: Create community-clinical linkages and improve education, policy, built environment, and access to quality care in preventing and managing chronic diseases. Guiding Principles include Collaborative, Communication, Data-driven, Equitable, Innovative, Respect, and Work-life Balance. Overarching Goals are Healthy People, Healthy Communities, and an Equitable Society. HEAL emphasizes partnerships with staff, partners, and community organizations to improve population health outcomes and to support policy development and environmental changes that enable healthier choices and better access to care. The program also maintains data and reports resources, supports CHW state certification information, and promotes nutrition, physical activity, and healthy living while focusing on health equity and reducing chronic disease risk.
